Linda's book has a form for every occasion and is an invaluable resource.

I make a master list of my students on each form at the beginning of the year and leave some blank ones. Then I run them off on different colors of paper so they are easy to find. I always have what I need at my fingertips. I file them in a binder and so everything is very organized when I go to write report cards. It’s a real time saver. I found Linda's student and parent forms very helpful for gathering information and feedback too - clear, straightforward and interesting. Students love to complete their self-evaluation forms, and it helps them become more aware of their own behavior and strengths and weaknesses.

I served as principal of South Park School for many years. During that time Linda developed forms that many teachers used for their own record-keeping. She also developed self-evaluation forms for students. I read all of the report cards before they were sent home, and I was very pleased when I saw these forms included with the teachers’ reports. They told me a good deal about the individual students, and I know that completing the forms was very beneficial to them.
